I'm using vanilla Android 5.1.1 on a Nexus 7 2013 tablet with Timur's ROM installed. The car is a 2008 Subaru Liberty (Legacy). It does not use CANBUS. I have high-speed polling enabled (but the problem exists under low-speed polling as well). Basically I have Torque Pro running all the time.

When I shut the car down, the tablet goes to sleep. When I start the car again, Torque Pro is the foremost app, connects to the car and starts showing data again. This worked perfectly with the Bluetooth version of the OBD-II reader.

Unfortunately this is not the same for a USB reader I installed to get more real-time data without any transmission delays. What happens is that Torque Pro appears to re-scan for devices when it comes out of sleep and adds it to a cached list of discovered readers, but instead of wiping the original list, it simply appends it, thus creating a list of detected USB devices that don't actually exist.

The end result is that Torque Pro tries to connect to a USB UNID that doesn't exist and thus cannot talk to the car. The only way to fix the problem is to quit Torque Pro and then restart it, or to remember to quit Torque Pro before turning the car off so that when I next turn on the car and Torque Pro automatically launches when it detects the OBD-II reader, it connects properly because it's the first instance. I'm sure it's an easy fix - this video is simply to demonstrate to the Torque Pro author what the problem is.
